Please take this survey once for each grade span (elementary, middle school, high school) that you have had a child complete this past June. For example, if you have two children, one having completed 5th grade and one completed 10th grade, you could complete the survey twice: once in relation to the grades 3-5, and once in relation to the grades 9-12.
